This is from memory, so it may very well be wrong, I encourage anyone to correct me.

Intro:

Bb/C  >  Cb/Db

Lead in:

Gb > Db/F > Ebm   : ||

Chorus:

Gb > (Bbm) > Ebm,  B > (Ebm) > Abm   : ||

Verse:  Cb/Db

Notes:
-All minor chords can be minor sevenths, the I and IV can be major sevenths.  The Cb/Db chord might have some extensions in it like a sus4, I dont recall.
-The chords in parenthesis are passing tones
-Cb is a funny name, but if you're in Gb, that's what you call it, because you already have a B.  If you're in F# then the chord becomes B/C#
-The guitar doesnt hit all the chords the bass does.  In the chorus, I think it plays the same chord over Gb and Ebm, and then the same chord over B and Abm.  Except for the root, those chords are the same.  I dont think the guitar plays in the "lead in" but I could be wrong.

I wrote the chord symbols and I left out most of the "extensions".  So for Gb you would play a Gb major chord, which has Gb Bb and Db.  The chords with the slashes means you're playing one chord over a different root, i.e. Bb (Bb D F) chord over C root.  You can try and play the C on the guitar, or just play a Bb chord and let the bass play the C.

I'm not sure what else to explain.  Does that answer your questions?

I played the song at rehearsal last night using these chords and they sounded ok.  I still havent heard the recording since posting this so I'm not positive they're correct.

E--1--
B--3--
G--3--
D--3--
A--3--
E-----

What you're doing is playing the Bb chord then adding the C into the Bass line.  So Just Form the chord like you always would and look for the Closest corresponding C in the Bass line which in this case happens to be on the A String 3rd fret.

Remember, in a Bb/C (for example) C is the root.  The bass plays the root (in most cases).  So if you like, you can just play a Bb chord.  Of course if you're like me you'll want to play it complete.

You can barre and "A" chord and raise the "A" string a whole step . . . or barre an "E" chord and raise the low "E" string a whole step.  Penguin's tab is for the former.
