Chord creation, Please?(how would yall play this?)

Ok guys, I was watching one of those church programs that come on tv and they were singing a song that I became interested in. I dont know if this is just one of those worship hymns or if an artist has recorded this. Nevertheless, I would like for someone to create some chords for me to play this song. I'll put notes by the words so yall will know how it goes. First off, let me say that the tempo is slow like "praise is what I do" or "becuase of who you are". You know, one of those type songs. Anyway, it goes like this:

Man, I finally used them chords tonight at church and I just had to thank you one more time. I mean, the spirit was movin and I hit them chords and sang the lil song that goes with it and it just fit perfect! It was one of them moods where you have that feeling in your stomach and just wanna cry even though nothings wrong.  Truly, God was in that place tonight and those chords you gave helped me a lot!
