God Bless you everyone. I am a minister of music and in this last month of the year I want to give out a Ministry Evaluation Questionaire to each member to fill out. Because I'm new to the position at this church, I feel that this is a good way to see how the people who serve in this ministry feels about the current state of the music department. I am asking you guys for possible questions I can put on this questionnaire and in what format should I ask these questions.

Are you looking for questions for the MOM, choir members, etc....or Ministry in general.

Here'a a few questions that I was asked.
Are you saved and filled with the Holy Ghost?
What is your purpose of being a MOM...is it for the money, for the passion, or for the heck of it...LOL
What are your short term and long term goals for your ministry?
What do you expect from your leaders? The music department, pastor, etc can tell you what they expect from you but you need to tell them what you expect from them. This will eliminate any future confusion.
Have you ever held this position before, if so, what was the reason of your departure?
