How can i Decode this to something like C/FAC

Do some of the heavy lifting -

Eb chords - every other note in the Eb scale - Eb F G Ab Bb C D Eb

So -  LH Root note/ RH notes
Eb =     Eb         Eb G Bb
Ab =     Ab         Ab C Eb
Bb =     Bb         Bb D F

Ebm =     Eb         Eb Gb Bb
Abm =     Ab         Ab Cb Eb
Fm =      F          F Ab C
Cm =      C          C Eb G

For slash chords like Bb \ D, the first part is the RH chord, the note after the slash is the LH note.

This is most of the basic chords in this song. For instructions on adding 6/9, etc. see T-Block's link.

Once you have done this, go up one letter (like Eb to F) to transpose to the key of F...
