Hi all,

     I've attached a simple year-end feedback survey we used for out choir members to fill out - takes 5-10 minutes, avg. - most of the questions are (check one), 3 are fill-in-the-blank...

Is there an additional page?  I only see "check one"s

Oops - (Embarrassed)  :-[  that was my own results I uploaded, by mistake.

Attaching a blank form - only one page, mostly check one w/a few fill-in-the-blank questions...
