I just got that CD and was picking out that song, I hear it a bit different.

The main verses are essentially a capella with a rhythm guitar choking on the chord, which sounds like a standard E:

E
I am not forgotten, I am not forgotten, I am not forgotten
                              B   E
God knows my name
(repeat)
                   E      F   F/D (or a Dm, whichever you prefer)
He knows my name    (vamps on the E  F  F/D til back to top)

E
Light over darkness, strength over weakness, joy over sadness, He knows my name
E
Father to the fatherless, friend to the friendless, hope for the hopeless, He knows my name

[walk down to bridge with E  Eb  D  C#  C]

Em/C
I will praise You, Lord, I will praise You, Lord
C/A                                    B
I am fearfully and wonderfully made

[That's the fast part of the song.  The slow part you mentioned is the following:]

D/B    A/F#  D/B    A/F#
E/C#  B/Ab  E/C#  B/Ab

D/B     A/F#      D/B  A/F#
I   am   not  forgot -  ten
E/C# B/Ab     E/C# B/Ab
Ne  -  ver   forsa  -  ken

Hope that helps
