CAN SOME CHORD THIS OUT THE REGULAR WAY

It just sounds like a standard generic traditional gospel/blues groove in the key of C to me. But I'm not a pianist, so maybe it's more complicated than I think. I think I understand the way people "tab" out piano chords, and I've got my ukulele on hand, so I'll try to help.
It'll be simple chords, but you can add your own flavor. I'll put in some lyrics here and there so you don't get lost.

Song is in the Key of C.

I'll skip the intro.

Chord chart legend:

*/*,*,* = Chord Symbol/Left-hand bass line (Why do you guys put the left hand part on the right side?)
If I can't find the bass note, I'll leave it blank.

First phrase:

C/C,C,D,D,G,G,A,C(almost the same as the first, but you jump from A - C in the last two bass notes)

Then you bridge to the next line with a run:

Wash rinse and repeat. The thing they do at the end of the song is just the first phrase over and over.

I hope you're familiar enough with the song to make sense out of this. Hope it's good enough... my first time doing piano-esque style chords. It's a pretty straight forward gospel song. You can add your own runs and riffs and whatever-you-call-it.
