what is the piano chords on this

  Look at the first chord  5       the 1=Bb; 3=D; 5=F  this forms a Bb major chord in root position.
                                  3
                                  1

This chart is based off of the Bb major scale's degree.
1 = Bb
2 = C
3 = D
4 = Eb
5 = F
6 = G
7 = A

Keep in mind, you still have to listen to the song. Just in case you are wondering why I didn't write out the letter names, this allows me to transpose parts if a song is too high or low for my choir and its easier to write to me.
