Sight-reading is the reading and performing of a piece of written music, specifically when the performer has not seen it before, also called a prima vista

I can sight read but at present only simple pieces
I can also sight read most intermediate pieces but not at  an acceptable speed to perform in public.
I can also read music from a hymn book and convert it to chord form and play it.

Hoping to move forward and improve on this in 2015 & I am currently studying a course

I can sight read a chord chart with no problem. I can sightread simple notation. Sight reading advanced pieces is just not a need right now.

I can sight read simple pieces, but not intermediate or difficult songs proficiently enough to play live. But when I have been caught in that situation, I would simply sight read the bass line (simple since its only one note) and use my ear to anticipate the upcoming/corresponding chords based on what has been played. I'll try to keep an eye out for any weird chords or accidentals that may come up in the right hand, but for the most part thats how i cheat my way through it.

Yep...

Mainly classical as I'm still doing lessons/exams.
Also hymnal music that I usually rehamonize.

But I still mainly play by ear.
