Hmmmm...maybe the intonation is off a little on you guitar,if you play a Fender type guitar. Some guitars don't have a bridge that requires intonation screws. These are the screws on the bottom of the bridge where the strings go.You may have to take it to a guitar tech and have him set it properly. You can also try different strings too if you want, sometimes that helps.Maybe also the way you put on strings, too. Not critizing or anything, just trying to gap the holes here.

1. Are you winding the strings around the tuning post correctly? For me, no more than 2 wraps around the post and then through the hole.
2. Are you properly stretching the strings when they're new? I always pull up on the strings near their middle point when they're new to stretch them and remove slack from around the tuning post.
3. Is your neck joint/screws tight? Assuming you've got a bolt-on neck guitar, if the neck screws are loose it will shift and you'll be out of tune.
4. Are the tuning machines tight and do they spin without slipping or excess friction? Cheap tuners will prevent any guitar from staying in tune.
5. Are the bridge saddles tight and do they have the proper spring retainer? If they slip then you'll be out of tune.
6. Is the intonation set? The 12th fret harmonic should match the 12th fret fretted note. If the fretted note is sharp, move the bridge saddle away from the neck. If the fretted note is flat, move the bridge saddle towards the neck until the harmonic and fretted note are equal.
7. Is the nut tight and are the nut slots cut correctly? A bad nut will cause out of tune issues by either slipping or catching the string.
Music theory is not always music reality.

And this kind of summarizes the majority of things that have been said already . . . have you had your guitar set up recently?  That's when they check the action, intonation, fret wear, tuners, connections, etc . . . clean, polished, and change your strings.  Unless you know your guitar is in perfect shape (and can adjust the action yourself as climate changes) I would recommend taking it in for a set up at least once every 6 months . . . I take mine in more often than that, as mine sees a lot of heavy playing, travel, and climate chances, and I dont want to trust it to my own hands . . . should cost $25-50 plus new strings.  Well worth it.

I did a live session one time where I was having trouble tuning my guitar beforehand . . . turns out in the week of travel and rehearsals my action had gotten knocked badly . . . I learned my lesson then!

Put some Graphite (pencil led shavings) in the sting slots of the nut. You'll stay in tune much better :)
