practice theories?

Does anyone have a legitimate practice routine that is designed to improve your playing? I know everyone says to practice everyday and I definitely make it a point to do so. But how do I maximize each practice session so that I can progress naturally in my playing. Thanks for your input.

Will:

Write down what you are practicing each day. You may find out that you're not practicing enough or where you should practice more.

For instance:
Monday Practice - C and F scales and chords
                           New praise and worship song
                           2-5-1 Progression

Tuesday Practice - Bb and Eb scales and chords
                           Songs for chior rehersal.
                           1-4-5 Progression

and so on ....
